The Herbert Protocol is a form that family members or carers complete for a person living with dementia or someone who may be at risk of going missing. Download a Herbert Protocol form from Police Scotland website.
The form provides personal details, medical history, significant locations and information about previous incident of going missing, and can also be used together with Alzheimer Scotland’s Purple Alert.
Family, friends and carers download the Herbert Protocol, which can be printed out and stored in a safe, easily accessible place in the person’s home or kept by a family member electronically.
