Citizens in North Lanarkshire are using innovative digital technology to enable them to self-manage their own health conditions. The use of technology is changing the local culture of accessing local services from one of dependency to one of empowerment, and is also resulting in improved outcomes and empowered citizens.
By using a co-production approach with local people and partnership staff, North Lanarkshire HSCP has remodelled service delivery and enhanced integrated working across the local health and social care system. Future developments are planned that will meet local and national policy.
This workshop will use a table top model and technology will be provided to allow participants to fully explore and access North Lanarkshire’s self-assessment and management system. Delegates will be provided with case studies to enhance their thinking about early intervention and prevention, and integrated working. Resource implications will be examined and evidence demonstrating good practice will be provided.
